Participating providers must pass strict credentialing criteria prior to joining the network.  There is no limit to the number of times you can access a participating provider in this program. Since there are no age or medical history restrictions, anyone can use this benefit. The networks provide discounts on health care services at physician's offices. Because the program is not insurance, there are no claim forms, deductibles, or waiting periods. The network is effective and easy to use. Hospital Referral Network

The Hospital Referral Network is made up of over 2,000 of the finest medical facilities in the country.  You have the opportunity to save hundreds, even thousands of dollars, on your hospital stay.  Remember, this program isnot insurance and you are responsible for paying the discounted bill in full. Travel Assistance Plan

As a member, you have access to a worldwide network of medical and travel assistance personnel whenever you travel.  Travel Assistance and Emergency Medical Evacuation Services are available when you are traveling over 100 miles away from home and you are faced with a serious injury or illness or a travel related emergency.  All medical services are provided based on medical necessity, which is determined by Lifeguard's Program Medical Advisor in collaboration with the eligible member's attending physician.

Assistance Services are available 24 hours a day, 365 days a year, worldwide.

Emergency Medical Evacuation.

In the event a Participant suffers from a Certified Illness or Injury that requires medical transportation of that sick or injured Participant, Lifeguard's Assistance Provider will provide medical transportation from the attending Hospital to the nearest appropriate Hospital capable of providing such treatment. Such transportation will be provided upon certification by Lifeguard's Assistance Provider's Program Medical Director, liaising with the attending Physician, that the Participant's severity of Illness or Injury necessitates medical transportation to the nearest appropriate Hospital.

Medically Necessary Repatriation.

In the event Lifeguard's Assistance Provider's Program Medical Director, liaising with the attending Physician, determine it is medically necessary to bring the Participant from the attending Hospital to a Hospital or nursing home nearer his/her permanent residence, subsequent to receiving Emergency Medical Evacuation Services provided under this Agreement, Lifeguard's Assistance Provider will provide flight by common carrier or air ambulance, and ground ambulance, if necessary, by the most direct and economical route.

Transportation of Mortal Remains.

In the event of death of a Participant, caused by a Certified Illness or Injury, Lifeguard's Assistance Provider will arrange to have the Participant's remains returned to a location nearest the Participant's Permanent Residence, including but not limited to, preparation, transportation and container for transportation. 

Transportation of Escort.

In the event a Participant requires Emergency Medical Evacuation by air ambulance or  Repatriation by commercial airline provided in this Agreement, air transport of the Participant's Spouse or other Family Member or traveling companion will be provided so that person may accompany the Participant in flight, subject to space availability, giving priority to medical equipment and medical personnel aboard and for the welfare and safety of the Participant receiving Services in this Agreement.

Family Visitation.

In the event a Participant, who is traveling alone, is expected to be Hospitalized for more than seven (7) days, round trip economy transportation on a regularly scheduled airline or equivalent will be provided for one Family Member or relative from the commercial airport nearest the Family Member's Permanent Residence to the Hospital where the Participant is confined so that Family Member may attend the Participant during the Hospitalization. 

Minor Child(ren) Return / Escort.

In the event a Participant's Minor Child(ren) are left unattended as a result of a Participant's Emergency Medical Evacuation, being Hospitalized for more than 24 hours due to Illness or Injury, or in the event of  death, one way economy transportation on a regularly scheduled airline, or the equivalent, will be provided to return the Minor Child(ren) to the Participant's Permanent Residence. A qualified attendant will also be provided by Lifeguard's Assistance Provider to accompany the Minor Child(ren) under the age of 16 when necessary. 

Vehicle Return.

In the event a Participant should suffer from a Certified Illness, Injury or death which requires Emergency Medical Evacuation/ Medically Necessary Repatriation or Transportation of Mortal Remains and the Participant is thereby unable to drive his/her vehicle, Lifeguard's Assistance Provider will provide vehicle return service for ground vehicles such as cars, trucks, vans, travel trailers or motor homes, operated by the Participant, to the Participant's Permanent Residence. This benefit will pay the cost, up to $1,000 for fuel, oil, driver and tolls to affect such vehicle return.  The Participant will bear the cost of any repair due to mechanical breakdown en route, as well as, costs for food and accommodations during the trip. The vehicle must be in a condition capable of being safely operated on the highway.

24 Hour Information Service.

Should the Participant need information before and/or during travel he/she may call Lifeguard's Assistance Provider 24 hours a day to obtain help.  The multilingual staff is prepared to assist and coordinate the management of a wide variety of travel related situations. Services include but are not limited to information on required documents, immunization requirements,  State Department Travel Advisory warnings on travel to certain locations, weather and hazard information about foreign locations, suggested medical exams or treatment before departure and medical care enroute.

Medical Monitoring.

Should the Participant need to be medically monitored, Lifeguard's Assistance Provider's duty Physician will monitor the case while liaising with the Participant, the local attending Physician, the family Physician and the medical director of Lifeguard's Assistance Provider.

Medical Referral.

Should the Participant need help locating a Physician or Hospital, Lifeguard's Assistance Provider will provide referrals to a local pre-qualified Physician and/or Hospital.

Guarantee of Medical Expenses.

Should the Participant need help for overseas medical payments, Lifeguard's Assistance Provider will arrange for payment or guarantee of payment to medical providers.

Insurance Coordination.

Should the Participant need help for overseas medical claims, Lifeguard's Assistance Provider will assist him/her in coordinating the claims procedure with the Participant's insurance program.

Lost Document Service.

Should the Participant need help to replace lost or stolen travel documents (i.e. passport, baggage, tickets, credit cards, etc.); Lifeguard's Assistance Provider will advise and assist where possible regarding their replacement.

Legal Assistance.

Should the Participant need help arranging local attorneys, embassies and consulates, arranging bail, or coordination of payment for legal Services, Lifeguard's Assistance Provider will provide referrals and payments, from available resources of the Participant.

Emergency Delivery of Prescription Items.

Should the Participant need prescription medication or lenses not available locally, Lifeguard's Assistance Provider will organize the delivery of the prescribed item to the Participant upon written authorization from the prescribing Physician when possible and legally permissible.

Emergency Cash Transfers and Advances.

Should the Participant need cash as a result of  loss or theft, Lifeguard's Assistance Provider will arrange for emergency cash transfers and advances through additional sources, including hotels, banks, Consulates and Western Union, up to a limit of $500 per transaction.  All transactions are subject to any government regulation and to the availability of the Participant's confirmed personal credit.

Language Assistance.

Should the Participant need help communicating in a foreign country, Lifeguard's Assistance Provider will provide telephone interpretation.

Roadside Assistance Plan

As a member, emergency road service is available 24 hours a day anywhere in the United States and Canada.  Whether you have a flat tire, a dead battery, or lock your keys inside the vehicle, assistance is just a phone call away.  Membership covers you, your spouse and any legal dependants 23 years old or younger.  

Services.

The Emergency Road Services  are limited to one service call per disablement for a service provided by a commercial garage, locksmith, or werecker service to:

  • Tow the vehicle
  • Change a tire
  • Boost the vehicle battery
  • Unlock the vehicle
  • Replace a lost vehicle key
  • Free the stuck vehicle
  • Deliver gasoline to the vehicle
  • Deliver parts for the vehicle to the disablement site

Contingency Fee Discounts:
Attorneys often work on a contingency fee basis on cases such as personal injury and collections. This fee is usually expressed as a percentage of the amount collected or awarded. In some cases, fee maximums are set by state law. In these matters, member's fees will be a 10% reduction of the state maximum rate or the attorney's usual rate, whichever is lower.

Guaranteed Fixed Hourly Rates:

In the event a member requires extended legal representation, our plan member attorneys have contracted with us to never charge more than $75.00 per hour for in or out-of-court representation. In most cases, the attorneys will be able to estimate the amount of hours any given case may consume prior to beginning work on the case.
Voluntary Rate Sheet

Each of our Financial Professional has, at a minimum, a college degree and must earn the designation of Certified Financial Planner (CFP) or an equivalent professional designation. Each individual has excellent people and listening skills and diverse professional backgrounds. They are trained to ask questions and identify the key issues the caller needs to consider when making an important financial decision.

REPORTING:

You can receive periodic reports on usage and general types or trends in inquiries across your participant population. We do not release information about individual participants.

This program does not include the provision of investment advice or the opportunity to meet face-to-face with our professionals. Nor does it include individual research on issues or preparation of direct correspondence. When requested by the participant, the professional may refer the caller to an outside professional or a not-for-profit agency for additional assistance. Attorneys often work on a contingency fee basis on cases like personal injury and collections. This fee is usually expressed as a percentage of the amount collected or awarded. In collection matters, your attorney will accept 18% if the case is settled before formal court proceedings begin. After proceedings begin, the fee is 27%. On all other contingency matters, there is a 10% discount on the lower of either the state maximum or the attorney's standard rate.

Patient Advocacy

Lifeguard offers a Personal Patient Medical Advocacy service that is dedicated to helping people with their medical bills. We focus primarily on those who are Un- or Under-insured by providing them a safety net.  Over the last seven years, we have worked with large groups and thousands of individuals nationwide, saving them millions of dollars.

Over 60 Million people are Un- or Under-insured.  Many of them are not able to take provided with health insurance benefits through work for a variety of reasons including:

  • They are self-employed, or, if an employee,
  • The employer offers little or no health care benefits.
  • If the employer offers benefits, employees opt out because they can't afford the premium.
  • Employees leave their families off the plan because they can't afford the additional cost.
  • Employees are part-time or non-salary.
  • Employee or family member has a pre-existing medical condition.

Our approach to Patient Advocacy is "hands-on high touch".  Patients have direct access to our Patient Advocates, once referred by the program they are a part of, to help them deal with larger balances.  Even with discounts available on physician and other health care services, people often are still unable to pay their medical bills. Many will put off recommended treatment because they don't have the financial resources to pay the balance of the discounted amount at the time of service.

Patient Advocacy from the Karis Group is available to help with all medical bills, which total greater than $5500 (or higher, depending on bill threshold chosen by your program) at any one provider. Our staff guides patients through the tangled maze of the medical bill claims process.  We also research the availability of government and private entitlement programs, grants, foundations and other payors to attempt to locate other sources of funding to pay the bills, as well as negotiating discounts and payment plans where appropriate.  We can help patients with all aspects of their medical bills, from securing discounts before the doctor is seen, to coping with demands for payment after bills have been incurred. 

Our goal is to guide each patient through the process of dealing with hospitals, doctors, clinics, labs, billing companies, collection agencies, governmental agencies etc., so that they can responsibly resolve their medical bills.
